In order to enroll in one of the retreats, there are some general and medical questions to answer in advance. Thank you for your openness.

1. Have you ever taken ayahuasca or other psychoactive substances? (If so, which substance and when)

2. Are you currently being treated by a doctor or therapist? (if yes: for what?)

3. Do you use drugs or alcohol regularly (if so, how often and how much?)

4. Are you currently taking any medications? (If yes: what is the name of the medication, what dosage, how often and since when)

5. Do you have any special eating habits, diets or other? (e.g. vegan, gluten-free...)

6. What is your concern, your reason, your motivation for coming to a retreat with Ayahuasca?

7. Do you currently feel physically and mentally healthy?
